Step #7: Notice of support from unit director, compliance safety officer or others who can confirm the project.

(Applicants without at least two reference statements will not be considered.)

Include at least two and no more than five written reference statements from individuals who can corroborate that the innovation is the laboratory or teaching setting (must be signed with contact information).

The statement should comment on the innovation’s applicability and effectiveness in managing safety. Appropriate references include supervisors, colleagues or other individuals with knowledge of the innovation and the unit compliance officer. Reference statements should be no more than two pages in length. Reference statements should contain the signature of the individual making the statement and his/her contact information (telephone number and e-mail addresses).
